Abilities United provides a lifetime of services to individuals with developmental and physical disabilities. Back on Track to Success is a community-based mentoring program at Santa Clara Valley Medical Center, focused on helping teens and young adults with disabilities to get a job or go to college. It is a one on one mentoring program, where mentors are trained to connect participants with the appropriate community resources and support that they need to be successful.
